Understanding Your Health Choice Coverage
Health Choice is a managed care organization (MCO) that administers Arizona's Medicaid program — AHCCCS (Arizona Health Care Containment System) — for eligible residents across the state. As a Health Choice member, you receive comprehensive health coverage at little or no cost, including benefits for substance use disorder treatment and behavioral health services.

Health Choice serves Arizona residents who qualify for AHCCCS (Medicaid). You may be eligible if you meet any of the following:

Does Milestone Recovery accept Health Choice for all programs?
Milestone Recovery accepts Health Choice for many of our core programs, including detox, residential, PHP, IOP, and outpatient services. Coverage specifics depend on your individual plan and clinical needs. Our team will verify your exact benefits before admission so there are no surprises.
Will I owe anything out of pocket?
Most Health Choice members have little to no out-of-pocket costs for covered services. Our billing specialists will review your benefits in detail and be transparent about any potential cost-sharing before you begin treatment.
Do I need a referral to come to Milestone Recovery?
In most cases, you do not need a referral from a primary care physician to begin substance use treatment. However, prior authorization from Health Choice is typically required. Our admissions team handles the authorization process for you as part of your intake.
What if I'm not sure I have Health Choice?
No problem. Call our admissions line and have your AHCCCS ID or date of birth available. We can quickly look up your insurance information and determine your coverage. If you're not currently enrolled, we can also connect you with resources to apply for Health Choice.
What happens after I complete treatment?
Recovery is an ongoing journey. Milestone Recovery provides robust aftercare planning for all patients, including connections to outpatient services, sober living, community support groups, and other resources — many of which continue to be covered under your Health Choice plan.
